Package Crystal

The LUX MED Group has prepared the Crystal medical programme covering a wide range of examinations and specialist consultations and an attractive discount for services not covered by the programme. The medical programme is intended for any person wishing to take good care of their health.

The offer encompasses the recommended medical consultations as part of compulsory prophylaxis for the given age group, as well as an extended scope of specialist consultations and diagnostic examinations. Terms of joining the individual programme Crystal:

• The programme is intended for persons aged below 65.
• The contract is signed for a minimum of one year and after that time it may be terminated with 30-day notice. If the contract is not terminated, it shall be extended indefinitely.
• Payments are made up-front for the entire year or in half-yearly instalments, via the Internet (by bank transfer or by credit card payment).
• Services under the package are provided at Medycyna Rodzinna and PROMEDIS facilities.
