LUX MED Patient Card
| Patient card is a document which entitles the Patient, after his identity has been established, to access medical services at all LUX MED Clinics around Poland /with the exclusion of Clinics located on the premises of companies/. Each card includes a unique Patient's identification number and their category of medical services, which LUX MED is obliged to provide under a given health insurance package. | ![]() |
| A Patient Card entitles its holder to access medical services, which are not rendered under a given health insurance package and provided by our Clinics at a 10% discount. Please give your card number each time you book an appointment and present it upon registration at the reception desk. The Patient Card may be used solely by the Patient entitled to medical care under this Card. | ![]() |
Lost or Damaged
The Card is the property of LUX MED. Please notify us immediately if the Card has been stolen or damaged at:
