Information on the Continuation Package:

The LUX MED Group has prepared the "Continuation" medical programme providing basic and extended medical care. The programme is intended for company employees and their families, whose medical insurance paid by the employer expired no more than 90 days earlier.
The offer encompasses, among others, specialist consultations, laboratory diagnostics and diagnostic imaging, contrast agents for examinations, outpatient procedures performed during specialist consultations, skin allergy tests, flu/tetanus vaccinations, and for women - pregnancy management without waiting period. Terms and conditions of taking up the individual Continuation Package:
• the programme is intended for persons aged below 65,
• the contract is signed for a minimum of one year. After that time, the contract may be terminated with 30-day notice. If the contract is not terminated, it is shall be extended indefinitely,
• payments are made up-front for the entire year or in half-yearly instalments, via the Internet (by bank transfer or by credit card payment),
• services under the Continuation "A" package are provided at own facilities of PROMEDIS and MEDYCYNA RODZINNA only, indicated on the www.promedis.pl website, during their operating hours.
• services under the Continuation "B" package are provided at own facilities of the LUX MED Group: LUX MED, CM LIM, PROMEDIS and Medycyna Rodzinna, indicated on the www.luxmed.pl website, during their operating hours.
